LifeSpa Panchakarma Terms & Conditions

Deposit
A 50% deposit is due at the time your PK appointments are scheduled to secure your dates and times.

Cancellation Policy
Given our financial commitment to our therapists, their commitment to being available for our Panchakarma guests, and because we turn away other patients if our schedule is full, we have implemented the following Cancellation Policy.

  • Cancellation 3 weeks (21 days) or more before your start date:  Your deposit is fully refundable only when notice of cancellation is given at least 3 weeks prior to the first day of your scheduled PK program.
  • Cancellation 20 days or less before your start date:  50% of the cost of the treatment(s) will apply if you cancel 20 days or less prior to the first day of your scheduled PK program.
  • Cancellation within 24 hours: 50% of the cost of the treatment(s) and/or cost of a consult with Dr. Douillard will apply.

Reschedule Policy
If you need to reschedule the dates of any of your treatments, kindly give us 14 day notice, or there will be a $25 reschedule fee per treatment.If you reschedule within 24 hours before your treatment(s), the fee will be 50% of the cost of the treatment(s) and/or cost of a consult with Dr. Douillard.

Form of Payment
Deposit payment may be made by MC/Visa/ Amex, cashier’s check or cash.Deposit payment may be made by personal check if made one month prior to your first scheduled PK appointment to allow for check clearance.

Balance Due
The balance of your PK bill is due by 2 pm on Friday or the last day of your program, whichever comes first.We accept MC/Visa/AMEX, cashier’s check, cash and personal checks.

Health Insurance
LifeSpa does not accept health insurance. By request we will supply a superbill that may be sent by the patient to your insurance company for re-imbursement. Note: Heath Savings Accounts are accepted forms of payment.

Confirmation
Please do not make travel arrangements until you have received definite confirmation of your Panchakarma program dates.

Gratuities
Gratuities are not expected.
